DESCRIPTION
Motilium (Domperidone) is an Rx medication that is used to treat slowed movement in the gastrointestinal tract associated with diabetes and gastritis (inflammation of the stomach lining). For these people, domperidone (dom-PAIR-eh-doan) improves symptoms of nausea (feeling sick), vomiting (being sick), bloating, and the feeling of fullness.
Domperidone belongs to the group of medications called dopamine antagonists and is also used to prevent stomach problems such as nausea and vomiting associated with certain medications used to treat Parkinson's disease. It works to improve symptoms by helping the stomach to empty more quickly and to reduce nausea.
Domperidone works primarily by blocking dopamine receptors found in the upper end of the digestive system. This causes tightening of the muscles at the entry to the stomach, relaxation of the muscles at the exit of the stomach and increased contraction of the muscles in the stomach itself. These actions speed up the passage of food through the stomach into the intestine, which physically helps prevent vomiting and reduces feelings of sickness, bloating and fullness. It also prevents food from flowing the wrong way through the stomach and so prevents reflux (regurgitation of the stomach contents back into the food pipe).
Domperidone also blocks dopamine receptors found in an area of the brain known as the
chemoreceptor trigger zone (CTZ). The CTZ is activated by nerve messages from the stomach when an irritant is present. It is also activated directly by agents circulating in the blood, for example chemotherapy medicines. Once activated, it sends messages to another area of the brain, the vomiting centre, which in turn sends messages to the gut, thereby causing the vomiting reflex. Blocking the dopamine receptors in the CTZ prevents nausea messages from being sent to the vomiting centre which, in turn, reduces the sensation of nausea and prevents vomiting.

SAFETY

Do not take domperidone if you:
- have heart disease such as heart failure, or a very slow or irregular heartbeat,
- have an abnormal heart rhythm seen on a heart monitoring trace (ECG) as a "prolonged QT interval" and people taking other medicines that can cause this problem - see below under ‘’Can Other Medicines Affect Motilium?’’,
- are allergic to domperidone or any ingredients of the medication, which are - The active substance is domperidone maleate,

- are taking the medication ketoconazole,
- are taking medications which cause QT prolongation,
- have bleeding in the stomach or intestines,

- have breaks in the lining of the stomach or intestines,
- have a prolactinoma (a tumour of the pituitary gland),
- have uncorrected levels of potassium, magnesium, or calcium in your blood,
- have cardiac disease (e.g., heart failure),
- have QT prolongation (a type of irregular heartbeat),
- have moderate or severe liver impairment.
Stop taking Domperidone tablets and see your doctor or go to a hospital right away if you:
- get swelling of the hands, feet, ankles, face, lips or throat which may cause difficulty in swallowing or breathing. You could also notice an itchy, lumpy rash (hives) or nettle rash (urticaria). This may mean you are having an allergic reaction to Domperidone tablets.
- notice uncontrolled movements. These include irregular eye movements, unusual movements of the tongue, and abnormal posture such as a twisted neck, trembling and muscle stiffness. This is more likely to happen in children. These symptoms should stop once you stop taking Domperidone tablets.
- have a very fast or unusual heartbeat. This could be a sign of a life-threatening heart problem.
Can Other Medicines Affect Motilium?
It is important to tell your doctor or pharmacist what medicines you are already taking, including those bought without a prescription and herbal medicines, before you start treatment with this medicine. Similarly, check with your doctor or pharmacist before taking any new medicines while using this one, to make sure that the combination is safe.
Domperidone may speed up the of absorption of certain medicines from the gut, for example paracetamol. This effect can be useful in migraine.
Domperidone may oppose the prolactin-reducing effect of bromocriptine and cabergoline.
The following medicines may reduce the breakdown of domperidone by the liver. This could increase the amount of domperidone in the bloodstream and hence increase the risk of its side effects. There may also be an increased risk of irregular heartbeats if these medicines are used together. Domperidone should not be used in people taking any of these:
- the azole antifungals itraconazole, ketoconazole or voriconazole,
- the macrolide antibiotics clarithromycin, erythromycin or telithromycin,
- protease inhibitors for HIV infection such as ritonavir, indinavir, nelfinavir, saquinavir,
- telaprevir or boceprevir for hepatitis C.
There may be an increased chance of an abnormal heart rhythm, seen as a 'prolonged QT interval' on an ECG, if domperidone is taken in combination with other medicines that can have this side effect, such as those listed below. Domperidone should not be used with people taking any of these:
- anti-arrhythmic medicines to treat abnormal heart rhythms, eg amiodarone, procainamide, quinidine, disopyramide, dronedarone, sotalol,
- certain antidepressants, eg citalopram, escitalopram, clomipramine,
- the antihistamines terfenadine or mizolastine,
- certain antimalarials, eg halofantrine, lumefantrine,
- certain antipsychotics, eg amisulpride, thioridazine, chlorpromazine, sertindole, haloperidol, zuclopenthixol, sulpiride, droperidol,
- arsenic trioxide,
- artemether,
- bosutinib,
- cisapride,
- delamanid,
- dolasetron,
- methadone,
- moxifloxacin,
- pentamidine,
- prucalopride,
- quinine,
- ranolazine,
- tolterodine,
- toremifene,
- vandetanib,
- vardenafil,
- voriconazole.
There may also be an increased risk of a 'prolonged QT interval' if medicines that can alter the levels of salts such as potassium or magnesium in the blood, such as the following, are taken in combination with domperidone:
- amphotericin,
- beta agonists - such as salbutamol,
- corticosteroids - such as prednisolone,
- diuretics - such as furosemide,
- theophylline.
The following medicines may oppose the stimulating effect of domperidone on the gut and so is likely to make it less effective:
- antimuscarinic medicines - such as antispasmodics (e.g. atropine, hyoscine, propantheline), medicines for urinary incontinence (e.g. oxybutynin, flavoxate, tolterodine, propiverine, trospium), or antimuscarinics for Parkinson's (e.g. procyclidine, trihexyphenidyl, orphenadrine),
- strong opioid painkillers, e.g. morphine, codeine.
